In some assignments in a courses, does not list all students' submissions according to the the number reported in the Grading list pager control.

In one observed case,
- the Assignments list shows: "133 /0"
- clicking on 'Grade', the pager control shows "Viewing 1 to 20 of 133 items"
-Click "Show 200 items."
- However, if you count the displayed items, there are only 132 listed
- If then you sort the list in descending order (Z - A) according to Student then the one that was not listed before is now listed on top. And the one that was on top, that is now suppose to be at the bottom is not shown.
- If you download all assignments you get all 133.

Similarly in another assignment in the same course. There are 55 submissions.
Click on graded and change view to Show 100. There are only 53 listed. Reverse the sorting order and the 2 not previously shown is now visible. And the top 2 has dissapeared off the bottom.
In this case is affects a student that showed up fine in the case above with 133 submissions i.e. it's not particular to 1 student.

We have no means to reproduce this, as we do not know what is causing it, but are happy to explore further in the database to identify the cause if required (and given some suggestion of what to look for).

Applied the change from SAK-4429 to our test server with a copy of the production db from today, and it appears to resolve the issues (r667). The equivalent change was also made in 2.2 so it should be fixed in there.

I believe this was the problem where students who had created draft submissions were showing up in the count in one place but not in another. That has been fixed - tested on 003. And there is a 2.1.2 fix as well.
