Survey Matrix Question response numbering is out of order

Student Responses for Matrix Question are not listed numerically. Order does not appear to be related to individual question settings (forced ranking, comment field). This same (incorrect) order is also shown in the Statistics page for instructors and students.

To Reproduce:

1. As instructor, create an assessment with several Survey - Matrix of Choices question
2. Have a student submit assessment
3. Click on the Submitted # link for the assessment
4. Go to the Questions tab
5. Click on the question links
