Description
After creating a question type, the pop-down menu for Insert New Question should reset to "select a question type". Instead the default pop-down selection remains the same as the last question added.
This means, to add another question of the same type one has to first select "select a question type" option and then reselect the question you want to add.
To reproduce:
1) Add a question type, let's use Multiple Choice. Save it.
2) Try adding another Multiple Choice question.
Expected behavior: the question type should say "select a question type" and you select Multiple Choice again and you get another Multiple Choice question.
Actual: the question type still says "Multiple Choice" which means you can not add a Multiple Choice without first explicitly selecting the "select a question type" option, then choosing Multiple Choice.
