Extended Delivery: After quiz has been published, it is possible to save an "empty" exception

      As an instructor:

      • Add an exception to a quiz
      • Publish the quiz

      On the Assessments page, select Settings from the drop-down beside the published quiz

      • Initial entry into exception setting, the button reads "Add an Exception"
      • Edit the existing exception, then click "Update an Exception"
      • Exception is put back into exception row and form is ready for new exception
      • The Exceptions button should now read "Add an Exception"

      Description

      Discovered while testing --SAM-3075--.

      When a quiz is saved, the button for adding/updating is correct.

      Once the quiz is published, opening settings displays the correct button text (Add an Exception).

      After editing an exception and updating it, the button text then stays "Update an Exception".

      Clicking Update an Exception without selecting user/group, allows empty row to be saved (no validation occurs).
