samigo: With multiple single selection (MC & Survey) questions on a page, ".undefined" displays in answer choices.

      1. In a site with at least one student, import the attached AllQuestionsExceptHotSpot.zip to Tests & Quizzes.
      2. Select Settings from the drop-down menu to the left of the quiz.
      3. Select Layout and Appearance and change the Question Layout to Each Part is on a separate Web page.
      4. Save Settings and Publish.
      5. Go back to Working Copies and Duplicate the quiz.
      6. In the new copy of the quiz, select Layout and Appearance and change the Question Layout to The complete Assessment is displayed on one Web page.
      7. Save Settings and Publish.
      8. Log in as the student and take the quiz that has each part on a separate page.
      9. Select the Table of Contents and navigate to the first question in the Multiple Choice part.  If issue present: You will see the .undefined in the answer choices.
      10. Exit the quiz and go to the other quiz with all questions on one page.
      11. Scroll down to the Multiple Choice questions and note If issue present: You will see the .undefined in the answer choices.
      12. Scroll down to the Survey questions and note If issue present: You will see the .undefined in the answer choices for the simple survey type question.

      Description

      If you create an assessment with more than one Multiple Choice, Single Selection or Survey question on a page, the answer choices for these questions display with .undefined (sometimes the word undefined is repeated several times, e.g. undefinedundefinedundefined).

      The additional undefinedundefined is also read by a screen reader, as shown with ChromeVox in this recording: https://www.screencast.com/t/AbrxFEshnlE6.  The issue makes questions extremely difficult to answer, if the student can handle answering them at all with all the confusing "undefined" repetitions.
