Add student name when instructor attaches file when grading assignment

Given a course site with student and instructor and assignment tool (English Language) Login as instructor and go to assignment tool Create an assignment that accepts attachments Create an assignment that's single file only Login as a student, Click each assignment and click "Or select files from home or site" Title of new window should indicate name of assignment and current student name, if the student select file(s) from home or site The "single file" should only indicate a single file Attach a file in both cases via any means (this dialog or the other) and submit Logout student and login as instructor  As instructor grade this student Click Add Attachments Note that the title of this window also shows the title of the assignment and the name of the student being graded. Previously all of the cases only had a static title saying "Add attachment to assignment" or "Add attachment"  (Optionally) Change the site language of the user The language probably won't include the new name and title additions unless it's updated but should display the original text Test an anonymous submission if this is enabled
