      1. Create an assignment that uses TII

      3. Ensure all files and the inline text have been queued with the content review service / is not in an error state

      Before the fix some items may not be added to the content review queue, depending on the order of the set (anything after the inline text submission was not added).

      Description

      The inline text associated to an assignment submission is added to the content-review queue twice.  It is queued on it's own and also added to the list of attachments, then the whole list of attachments is queued also.

      The problem is further exacerbated by SAK-41603, in which an exception causes subsequent attachments in the list to not be queued at all.
