T&Q: Attachment icon missing for png files

      1. Create a quiz
      2. Edit the default part (Part1) 
      3. Click Add/Remove Attachments
      4. Select a .gif and a .png file (notice icons are there)
      5. Click Continue

      Notice there is no longer an icon for the .png file

      Description

      Tests & Quizzes has an icon for a .png file when in the attachment screen, but once it's attached, there is no icon.

      There seems to be a difference between the icons for attaching in Assignments vs attaching in Tests & Quizzes also. It would be good if they were consistent.
