NPE for existing gradebooks using categories

After the changes in SAK-42414, existing gradebooks using categories will throw a stack trace caused by a NullPointerException. This happens because the is_equal_weight_assns column for existing categories will contain nulls. Newly created categories will have an appropriate true/false value.

I've created a conversion script that will set the nulls to false, and make the column non-nullable.
