Error messages do not take focus and might be difficult to find for some users

      In Tests & Quizzes as Instructor:
      1. Enter title to Assessment Title input box (ie test.)
      2. Press "Create"
      3. Choose from Add Question option box "Multiple Choice"
      4. Press "Save" at the bottom of the page
      Resulting "Question Text should not be empty." should be given focus as per "Errors on top" example in http://webaim.org/techniques/formvalidation/#usable. There also appears to be no need for this text to be in a table.
      Ideally aria-invalid="true" could also be set on the offending item, but I appreciate that might be a larger issue.

      "The error message should either be the first item within the new page or focus should be set directly to it. This allows screen reader and keyboard users to immediately access the error message without having to find it amongst the rest of the page contents."
